Format Tags Explanation
| <Title> | The person's title (Dr., Mr., etc.) |
| <Lastname> | The person's last name. |
| <LastInitial> | The first letter of the last name. |
| <MidInitial> | The middle initial. |
| <Firstname> | The persons's first name. |
| <FirstInitial> | The first letter of the first name. |
| <Suffix> | The person's name suffix (PhD, MD, Jr., etc.) |

If there is a single letter before the last name, that will go in the middle initial field, otherwise everything else will be considered the first name.
