Custom Internal Form (Existing Person)
Companion's internal forms allow for custom questions to be created to obtain information from the form. The Question Type determines the type of response allowed by the question. This page contains the details and requirements to configure each type of question format. Not all of the Question Types can be used for Internal Forms. Label, Header, Hyperlink and Embedded.
Client Forms:
- You have the ability to create unique Client Assessment forms
- Multiple forms can be added to each Client as needed
- Helps to track Client changes and progress
- Able to capture pertinent client information in one place
- Print, save or export as needed
- Examples include: Client Assessment Forms, Intake information, Home Assessments

Caregiver Forms:
- Ability to create multiple assessment type forms, to help evaluate caregiver capabilities and skills
- Customizable to your agency
- Ability to attach multiple reports to each caregiver as needed.
- Print, save or export
- Examples include: Supervisory Visits, Caregiver Skills Assessment Form
