The Employer Inquiry and Participant Application web forms are completely customizable, allowing you to ask the questions that are important to your office.

Once configured, a link to the form can be added to your company's web site to allow potential employers or participants access to the form.

First thing we have to do is create the topics and questions you want to appear on your participant application.

Setting up the Questions in User Defined

The topics and questions for the web forms are configured in the User Defined Module, Web Intake Questions tool.

Question Screen

The following field names and descriptions will assist you with populating the fields.

Field Names

Field Descriptions

Topic

A topic area that groups the questions. Questions from the same topic are grouped together under a heading of the topic name.

Question

The actual question text. (Required)

Question Type

Specify what type of response is expected from this question. This affects how the question is displayed on the page and the allowable response(s). The question types provided include:

  • String (text value)

  • Yes/No

  • [Whole] Number

  • Decimal

  • Date

  • Multiple Option (select multiple choices from a list of options)

  • Single Option (select one choice from a list of options displayed horizontally)

  • Single Option Vertical (select one choice from a list of options displayed vertically)
  • Label (text with no associated entry field)

  • Header (text with no associated entry field, displayed in a box with a title)
  • Hyperlink (ability to link to other pages or documents)

  • Embedded (embed multimedia content)

Order

The order the question will appear on the form within the topic group. If an order is not specified, or multiple questions within the same topic have the same order number, the questions are displayed in alphabetical order.

Question Footer

Additional text to display below the question, which can be used as additional information or instructions for answering a particular question.

Question Hint

Placeholder text that can appear in a text field on the form as a hint for answering the question, or a tooltip that can appear when hovering over any type of question.

Also, is used as a title for the question type Header. If no title is filled in the title bar is not displayed only the box appears around the text.

Option Item List

The list of selectable options for a Multiple Option or Single Option type of question.

Score

Specify a score value that may be assigned if the question is answered properly. Scores can be used on critical questions to pre-filter the application. If the total score of the application is less than the Minimum Accept Score on the application setup in the Agency Office Module, the application will be marked with a Rejected status. (NOTE: The score value may be positive or negative.)

Auto Reject

Automatically mark an application as "Rejected", regardless of total score, if this question is not answered correctly.

Image (Optional)

An icon to be displayed in list views within the program displaying questions/responses and will also show next to the question text on the web form.

Font Style

Apply special font styling to a question on the web page in order to make this particular question stand out. A blank selection is default/normal text.

Color

Specify a font color to a question on the web page in order to make this particular question stand out.

Indent (Spaces)

Indent the start of the question text a given number of spaces to the right on the web form. This feature is useful in conjunction with setting the Order to present a question as a sub-question or follow up to a previous question. (e.g. in follow up to a Yes/No question: "If YES, provide detailed info:")

Required

If checked, the applicant cannot save the web form without providing an answer to this question.

Remember using the Import button on the toolbar will allow you to import a collection of default questions and topics. You can then customize or add additional questions and topics as required.

Second thing we have to do is configure the Participant, Employer or Office Staff Application form.

Configuring the Application Form

The Employer and Participant web inquiry/application forms are configured in the Agency Office Module, Home tab, Web Forms Setup option.  This configuration determines the visual appearance of the form, as well as the questions and text that will appear. Each inquiry/application form is configured independently of the other.

Inquiry/application forms can be configured for Employers, Participants and Office Staff. Multiple additional forms can be set up under the "Additional Web Intake Forms" tab. (For example, you may likely wish to have different sets of questions for employees applying to different positions and save these applications under different categories.)

The following demographic information questions are always included in the form:

  • Name
  • Address
  • Contact information (Phone, Cell, Email)

The following fields on the setup form allow you to configure the appearance and questions:

Form Title

The title to be displayed at the top of the web page

Role

The type of person applying (Employer, Participant or Office Staff).

Category

Optionally choose a default category that the Inquiry/Application will be added as.

Instructions

Additional text to be displayed at the top of the form, below the form title. Use this section to provide additional information and instructions to your prospective clients or caregivers for completing the form.

Notification Email (Optional)

Specify an email address that will receive a message every time someone completes this form.

Minimum Accept Score

Certain questions can be assigned a score value that will be counted if the applicant answers that question. By assigning score values to key questions and specifying a minimum acceptance score, you can ensure that important questions are answered. If the total score of the application is less than the minimum acceptance score, the application's status will be marked as "Rejected." (This action will not prevent you from turning the "Rejected" applicant into a potential client or caregiver, but it does separate them from those who have passed the "Acceptable" score threshold.)

Show Office Name

Show the name of your office in the header of the page

Show Office Address

Show your office's address in the header of the page

Display your office's logo in the header of the page

Prompt for Languages

Display a question on the form that allows the applicant to indicate the languages they speak by choosing from the list of Languages in your system.

Prompt for Skills

Display a question on the form that allows the applicant to indicate the skills they possess (participant) or require for service (employer) by choosing from the list of Skills in your system.

Prompt for Certifications

(Participants Only) Display a question on the form that allows the applicant to indicate the certifications they possess by choosing from the list of Certifications in your system.

Questions

Select other questions you want to appear on the form from the list of Questions in User Defined.

Languages

Select the list of Languages to be shown as options to be selected on the form, if the Language question is displayed.

If nothing is selected in this list, then all of the Languages in the system will be presented as options to be chosen; otherwise only the selected Languages here will be selectable by the applicant.

Skills

Select the list of Skills to be shown as options to be selected on the form, if the Skills question is displayed.

If nothing is selected in this list, then all of the Skills in the system will be presented as options to be chosen; otherwise only the selected Skills here will be selectable by the applicant.

Certifications

Select the list of Certificates to be shown as options to be selected on the form, if the Certifications question is displayed.

If nothing is selected in this list, then all of the Required Dates in the system will be presented as options to be chosen; otherwise only the selected Required Dates here will be selectable by the applicant.

Legal Message

Additional text to be displayed at the bottom of the form, before the submit button. Use this section to place your employment application disclaimer and acknowledgement or applicant waiver details.

Submit Page Message

Additional text to be displayed on the thank-you page after submission of the application.

To see the form you have completed click the View Form button. A webpage will appear with your inquiry/application. The link in the address bar is the link you will want to embed into your company's website.

Completed Employer applications are found under the "Employer Inquiries" tool in the Main Module. Completed Participant and Office Staff applications are found under the "Web Applications" tool in the Human Resources tab of the Relationship Management Module.
