Caregiver

Caregiver Master

Summarizes all profile information about the selected Caregiver(s).

Birthdays

Show Caregivers’ birthdays. This report is disabled if the logged-in user does not have permission to view birthdays.

Referral Sources

Details Caregiver referrals.

Condensed Master

A two-column master report that contains limited information about each Caregiver.

Documented Events

Summarizes Documented Events in which this Caregiver has been involved.

Required Dates

Required Dates Coming Due

Show Required Dates that are or will be expired within a given date range.

Required Dates Coming Due Letter

Similar to the Required Dates Coming Due report, but in a letter format that you can mail to individual Caregivers. The body text of the letter is configured in the User Defined Information Module, Reported tab, Mail Merge Letter.

Required Dates Completed

Similar to the Required Dates Coming Due report, except this report shows which requirements have been completed within a given date range.

Schedule

Tasks Performed and Verification

Compares the tasks that were performed for a client vs. the tasks detailed in the Care Plan, to verify that the Client's Care Plan duties are being followed. The report details the tasks that should have been performed for the client (as detailed by their Care Plan) and compares them to the tasks that were actually performed by the caregivers (as captured on the schedules via Telephony task entry features or manual entry from timesheets).

This report can also show client signatures captured on schedules via the mobile app to verify tasks performed.

Caregiver Schedules

Show all schedules for the Caregiver(s) in a specific date range.

Schedule Expenses

Details extra charges attached to Schedules within a given date range.

Schedule Inactivity

Caregivers who are working less than a specified number of schedules within a given date range.

Schedule Gaps

Caregivers with large gaps between schedules on the same day.

Recurring Events

See all recurring events (Schedules, Tasks and Appointments) for the Caregiver(s).

Schedule Exceptions

Under/Over Max Hours per Week

Identify Caregivers who are scheduled more than their Maximum Hours per Week.

Under/Over Hours

Identify schedules where the Caregiver(s) worked more or less than what was scheduled.

Early/Late Arrival Time

Identify schedules where the Caregiver(s) arrived either early or late.

Weekly Overtime

Caregivers who have weekly overtime hours within a given date range.

Your office address and logo can be included using the report header options. A Note about the Caregiver’s Qualifications: Name badges display the Caregiver’s Primary Qualification. If a Caregiver has multiple Primary Qualifications, the report prints multiple badges for that Caregiver, one for each Primary Qualification.

Schedule

Care Plan

Details this Client’s plan of care. This report can include the Client’s general Care Plan information as well as details about their Authorizations and any Care Plan Duties specified on individual Authorizations.

The Care Plan report includes optional signature lines for the Client, their physician, the primary Caregiver, and your Office Staff person who is the Client Manager. The signed document may be kept on file as an official care agreement between the Client and your office.

Classic Care Plan

Classic version of the Care Plan Report, for existing users who may prefer this style. NOTE: This version of the Care Plan Report does not support the section specific Care Plan Notes.

This report has separate options for Portrait or Landscape orientation.

Form 483

Print this Client’s Form 485 Plan of Care (US Medicaid).

Expiring Authorizations

See Authorizations that will end within a given date range.

Over/Under Authorized Hours

View the clients who are over or under their specified Authorization Hours in a given period. Use this report to determine if your office is scheduling clients to their full potential.

Show Achievement statistics for the selected Office Staff. You can configure Achievements for each staff member in the Office Staff Module. Choose the goals to achieve and select targets to meet, and view the statistical results in this report.

E-TimeCard Edits

Show manual edit history made to E-TimeCards by Office Staff.

Telephony Instruction Cards

Print instruction cards for your Office Staff who use Telephony to track their hours.
